Dillyn Leyds has recovered from a shoulder injury to take his place in the Stormers' side to face the Cheetahs in Bloemfontein on Saturday.
The return of Leyds is one of three changes to the side that overcame the Bulls at Newlands, with prop Steven Kitshoff and lock Eben Etzebeth both being rested – the latter as part of the agreement with Saru.
Oli Kebble will make his first Vodacom Super Rugby start in Kitshoff's place, while Jean Kleyn will partner Ruan Botha in the second row.
Prop Ali Vermaak and lock Manuel Carizza have been drafted in as cover on the bench.
'We will have the same mindset for this weekend as we do for all South African Derbies. We understand how tough these games are and we know we need to pitch up,' said coach Allister Coetzee following the announcement.
'The Cheetahs pushed the in-form Lions in Johannesburg last week, they’re a good side – especially in Bloemfontein – and we know this will be a tough match.'
